Public Health Survey for Residents of Dover, Medfield, Needham and Sherborn

The towns of Dover, Medfield, Needham and Sherborn, are working together as the Charles River Public Health District (CRPHD), are seeking resident input about key public health needs in our communities.

It will take just 15 minutes of your time to complete the Public Health Needs survey. This survey will contribute key information about the people and resources in our communities, and help our health departments understand strengths, challenges, and needs.

The survey is completely anonymous and results will be used to make decisions about community public health priorities in each town.

Distribution throughout town and a high participation rate will help ensure that our results reflect all people in our communities. Therefore, we invite you to think of three or more people you could share the survey with who live in either Dover, Needham, Medfield and or Sherborn.
