Free Flu Shots

Saint Peter’s University Hospital Community Health Services will be coming to ACI to provide free flu vaccines.

Please bring to the appointment:

• Primary and secondary insurance cards
• A photo identification card
• The completed Adult Influenza Consent Form packet

SPUH NON-PARTICIPATING INSURANCES:

• AmeriHealth Commercial, AmeriHealth Local Value Network, AmeriHealth Hospital Advantage EPO
• Empire BlueCross BlueShield Commercial - unless it is a PPO plan, which is indicated on the card
• NaphCare

*For those who have a non-participating insurance, flu vaccination is available but will be billed to the participant*

Appointments are required. Walk-ins may be accommodated.
