Florham Park Rehab & Healthcare Introduces Chronic Illness Support Model

Florham Park Rehabilitation & Healthcare Center has launched a comprehensive Chronic Illness Support model, partnering with HealthDrive to integrate palliative care, pastoral support and other services with its specialized clinical programming. Subhra Sen, APN, a board-certified adult-gerontology and primary care nurse practitioner, is leading the Florham Park program for HealthDrive, bringing extensive in-patient medical experience and advanced training.

The patient-centric Chronic Illness Support model is available to clients of Marquis Health Consulting Services, which supports the facility. “Florham Park’s new program for chronically ill patients is truly a next-level model of care, and it is being supported by an excellent partner in HealthDrive,” said Marquis’ Erin Studt, regional director of market development. “There is great value in providing a holistic focus on both physical and spiritual wellness for patients.”     

The palliative care component of Florham Park’s Chronic Illness Support program is administered by a trained team of doctors, nurses and palliative specialists. The initiative focuses on providing expert medical care that includes emotional and spiritual support to manage chronic conditions and help relieve symptoms and stress. The goal is to enhance quality of life for patients and their loved ones.    

The new program at Florham Park is specifically supporting the facility’s specialty initiatives, including Pulmonary Care and Cardiac Care. It also weaves in disease-specific education and health coaching, multi-specialist led and coordinated care, social services support and connections to community resources and life enrichment activities.    

Located at 190 Park Ave., Florham Park Rehabilitation & Healthcare Center provides post-hospital care, short-term rehab and long-term residential care. The state-of-the-art facility’s three-story building is undergoing a top-to-bottom, multi-phase renovation and modernization. Phase 1 focused on the updating and remodeling of patient areas and has been completed. Phase 2 will introduce the largest rehabilitation therapy gym in the greater Morris County region.
