YCMAD Voucher Request Form Spay Neuter Vaccination Microchip JUST REMEMBER, we need to receive your application 5 days prior to your appointment date. Applications received with less than this time frame will NOT be Honored. Spay/neuter $5.00 copay; Vaccines DA2PP Copay $10; Microchip Copay $10. Total contribution for these services $25 Any additional service requested is not covered by this voucher The copay of $5-10 dollars covers only the cost of the surgery for dogs up to 50 pounds. Vaccines are only offered AT our collaborating clinic (DA2PP) and only when you request a spay or neuter voucher at a cost of $10.00. The cost of the required Rabies vaccine is NOT covered by this voucher. The clinic will not perform the surgery without this vaccine. We will not issue microchips under any other circumstance. Any additional service offered by the clinic and accepted by you is your responsibility.
